The Dean’s Certification Form is to confirm information about the disciplinary records of students originally provided in the electronic application. Please have your previously attended academic institutions submit this form no later than Friday, August 14, 2026.

Who can complete the Dean’s Certification Form?
This form should be sent to a department or a person at the institution that has access to students’ complete records. This can be the Registrar’s Office, Office of Student Affairs, Office of Academic Conduct, a program director or dean, or an equivalent department. The Office of Admissions and faculty members typically do not have the information needed to complete this form.

To which academic institutions should I send this form?
This form needs to be completed by all previously attended academic institutions, including those from which you did not obtain a degree.

Where should the completed form be sent?
The Dean’s Certification Form can be submitted directly to the Office of Admissions by email to graduateadmissions@law.stanford.edu. The form can also be submitted by mail to
